Models to Improve Retention, Adherence, and Self-Management
The HIV care continuum emphasizes the importance of helping people who are living with HIV/AIDS to stay connected to the healthcare system to improve medication adherence and reduce the risk of passing the infection to others.
People living with HIV need to continue antiretroviral therapy as prescribed in order to ensure proper disease management. Medication adherence is a major determinant of disease progression and health outcomes. However, adherence may be difficult for some people because of challenging life circumstances, including unstable housing or employment, substance use, mental health issues, or a lack of transportation.
Providers and other types of organizations can play an important role in helping people access and adhere to treatment.
