Making a Plan for Sharing Successes
Every program's dissemination objectives are different. The type of information being disseminated and the approach for sharing it vary depending on the intended audience. The questions below can help guide development of a dissemination plan:
- Research findings and products: What is going to be disseminated (e.g., findings from the program, evaluations of data quality, best practices, success stories, and products)?
- End users: Who will use the findings or products? This could vary and may include partners, other care coordination programs, or policymakers.
- Communication: What is the optimal model for sharing the findings? The mode for disseminating findings, products, or best practices will depend on the objective and the identified end user.
