Module 2: Program Models
The goal of care coordination is to ensure the delivery of high-quality and appropriate care across different healthcare settings. Different care coordination models can be implemented to achieve this goal. The most appropriate model depends on factors such as the needs of the patient, the relationships, and agreements between healthcare providers and organizations, the level of information sharing among healthcare providers and between healthcare providers and patients, and community characteristics.
