Evaluation Objectives
Care coordination programs should develop evaluation objectives that align with the evaluation questions. These objectives should also be SMART:
- Specific – Detecting changes in the program.
- Measurable – Collecting data that are complete and accurate.
- Actionable – Showing performance over time to identify when or where action should be taken.
- Relevant – Focusing on only the relevant indicators.
- Timely – Capturing data in the period it is needed, if not real time.
