Module 2: Evidence-Based and Promising Chronic Disease Management Program Models

Rural communities are implementing evidence-based and promising programs to manage chronic disease and improve health outcomes. There are different program models rural communities may choose for chronic disease management. Depending on the needs of the community, programs may implement or adapt one or more program models.

To learn more about how to adapt programs, see Developing a Rural Community Health Program in the Rural Community Health Toolkit.

This toolkit identifies eight evidence-based and promising models and approaches to address chronic disease management.
