Need: To increase access to healthcare throughout Alabama.
Intervention: The Alabama Department of Public Health (ADPH) has created telehealth communications at county health departments.
Results: ADPH telehealth services are currently available in 65 of 67 county health departments.

Need: To expand public health and human services to adults living in rural West Central Missouri.
Intervention: HCC Network organized Project Connect events, daylong resource fairs that offer free public health and human services to low-income adults, and now serves as a resource for other organizations who would like to host a Project Connect event in their area.
Results: In 2024, local Project Connect events welcomed 100 families and offered numerous services and resource connections.

Need: To reduce injuries in agricultural communities and improve emergency responders' preparedness when called to farms and ranches.
Intervention: RF-DASH equips rural fire/EMS personnel and others with agricultural health and safety knowledge and tools to pre-plan for agricultural emergencies as well as assess and then mitigate agricultural hazards.
Results: Over 250 firefighters and EMTs have received training to become RF-DASH trainers.

Need: To improve oral health among the residents of the Rosebud Reservation in South Dakota.
Intervention: St. Francis Mission founded a dental clinic, staffed by volunteers from across the country, to offer free oral healthcare to residents of the Rosebud Reservation.
Results: The clinic typically offered 20 one-week clinics every year and served 3,000 patients after transitioning to electronic records in 2021.

Need: To provide accessible and affordable services to address the challenges associated with aging, serious illness, and grief across rural western Colorado.
Intervention: A nonprofit, community-sustained healthcare model was created to provide the Program of All-Inclusive Care for the Elderly (PACE), dementia support, hospice care and palliative care, as well as grief support services for individuals of all ages.
Results: Since 1993, HopeWest has grown to serve more than 3,000 patients and families annually across five counties in western Colorado.

Need: To educate rural community members about Alzheimer's disease and other dementias and to support older adults with dementia and caregivers in need.
Intervention: A community group formed in Winnemucca, Nevada, to discuss topics like health, housing, social events, community improvement, education, and transportation for older adults. The group also provides outreach to older adults, caregiver support, and a variety of educational activities and events.
Results: The group's efforts have led to many changes for community welfare and safety as well as opportunities for education and activities.

Need: Clinicians in rural and underserved areas are often unprepared to provide comprehensive medical-forensic examinations for patients who present for care following a sexual assault.
Intervention: The MDPH TeleSANE Center uses secure telehealth software to connect sexual assault nurse examiners to clinicians and patients in hospitals across Massachusetts — including five in rural counties — offering expert clinical guidance and support before, during, and after examinations.
Results: Clinicians report that the service gives them increased confidence throughout the examination process. To date, the MDPH TeleSANE Center has assisted in the care of over 1,234 patients.

Need: Suicide among veterans has been steadily increasing, and rural veterans have an increased risk of death by suicide compared to urban veterans.
Intervention: A program called Together with Veterans was formed to help rural communities address and prevent suicides among veterans. The initiative is veteran-led, collaborative, evidence-based, and community-centered.
Results: Data collection is ongoing.

Need: Teen pregnancy, sexually transmitted diseases, and mental health challenges in adolescent girls were concerns for members of Union Parish, Louisiana.
Intervention: Union General Hospital, a Critical Access Hospital, created the program It's a Girl Thing: Making Proud Choices to teach prevention, self-confidence, personal responsibility, and mental well-being to teen girls.
Results: Teen pregnancy rates in Union Parish have dropped by more than 40% since the start of the program, significantly exceeding the program's initial goal of 5%. Graduation rates have also increased. The addition of Together We Can Be Bully Free as an integral part of It's a Girl Thing has further expanded mental health support for participants.

Need: Transportation to medical appointments for children and expectant mothers living in rural Alabama.
Intervention: Kid One Transport provides rides to medical appointments for clients who otherwise have no other means of transportation.
Results: The program has transported 477,880 children and expectant mothers to and from medical appointments. Children have recovered from life-threatening illnesses and are experiencing a higher quality of life.
