Telligen Community Initiative Grants
Grants for programs and nonprofit organizations that address social determinants that impact health and enhance equity, that are making healthcare education more accessible, and that are fostering healthcare innovation in Colorado, Illinois, Iowa, and Oklahoma.

Understanding the Intersection of Social Inequities to Optimize Health and Reduce Health Disparities: The Axes Initiative (R01 Clinical Trial Optional)
Funding for research to examine the pathways through which social determinants of health (SDOH), and related biological, psychological, and behavioral factors impact health and health disparities. The primary populations of focus are those that experience health disparities in the U.S., including racial and ethnic minority populations, people with lower socioeconomic status, underserved rural populations, sexual and gender minority populations, and people with disabilities.

Technologies for Improving Minority Health and Eliminating Health Disparities (R41/R42- Clinical Trial Optional)
Funding for small businesses to develop products, processes, and services that can be used to improve minority health and/or reduce and ultimately eliminate health disparities in one or more NIH-defined health disparity population groups. Technologies should be effective, affordable, and culturally acceptable.

Sierra Health Foundation Responsive Grants
Grants for communities and organizations in California that promote health and racial equity to address health disparities and the social determinants of health. The Foundation will prioritize projects that have been impacted by California's state budget funding gaps. At least 30% of the funding is set aside for rural communities.

Maine Community Responsive Grants
Grants for planning or implementation projects to improve access to care and health outcomes among communities and populations in Maine that experience health inequities, including rural areas.

Delaware Highmark BluePrints Grants
Grants for health-related projects, programs, and supportive services in Delaware in the priority areas of healthcare access, economic stability, social and community context, and neighborhood and built environment.

Northern New Mexico Health Grants
Grants for projects that promote the health and wellness of the diverse people of northern New Mexico. Immigrant, rural, and border communities are priorities.

Innovations for Healthy Living - Improving Minority Health and Eliminating Health Disparities (R43/R44)
Funding for small business concerns to develop a product, process, or service with the aim of improving minority health and/or reducing and ultimately eliminating health disparities in one or more NIH-defined population groups.

Blue and You Foundation for a Healthier Arkansas Regular Grants
Grants to organizations in Arkansas working to create better health for all Arkansans. Grant cycles in 2024 will focus on behavioral health, maternal and pediatric health, and social determinants of health.

Health Care Models for Persons with Multiple Chronic Conditions from Populations that Experience Health Disparities: Advancing Health Care towards Health Equity (R01 - Clinical Trials Optional)
Funding for research to study the effective adaptation, integration, and implementation of recommended guidelines of care of persons with multiple chronic conditions from populations that experience health disparities.
