AHRQ will provide funding to support research projects to advance the understanding of the role, capacity, and value of primary care to improve patient outcomes and population health by delivering whole healthcare across the life course, and to build evidence about the characteristics and value of primary care that influence patient outcomes and advance health equity.
Research projects that aim to improve primary care in terms of quality, safety, access, affordability, and equity will be given priority, including projects focused on:
- Primary care workforce
- Patient experience or outcomes
- Social determinants of health
- Care delivery models
- Payment
- Digital healthcare

Proposals focusing on issues that address health disparities and have the potential to improve health equity in terms of race and ethnicity, gender, socioeconomic status, age, disability, or geography/rurality are strongly encouraged.
Eligible applicants include:

- Public/state controlled institutions
- Private institutions
- Nonprofit organizations
- With 501(c)(3) status
- Without 501(c)(3) status
- Native American tribal organizations
- Faith-based or community-based organizations
- Regional organizations
- Governments
- State
- County
- City or township
- Special districts
- Federally recognized Indian/Native American tribal governments
- Indian/Native American tribal governments (other than federally recognized)
- Eligible agencies of the federal government
- U.S. territory or possession
- Other
- Independent school districts
- Public housing authorities
- Indian housing authorities
Award ceiling: $500,000 per year
Project period: Up to 5 years
