Program of All-Inclusive Care for the Elderly (PACE) Operational Readiness Grant Inactive
Funding to qualified organizations in Georgia to offset costs associated with meeting Program of All-Inclusive Care for the Elderly (PACE) organization operational requirements. Applicants must verify a firm commitment and intent to complete and submit the full PACE application required by the Centers for Medicare and Medicaid Services (CMS) prior to the end of the funding period.

Minnesota Live Well at Home Grants Inactive
Grants to develop and /or provide services to allow for older Minnesotans to live in the community. Funded projects will improve capacity to develop, strengthen, and integrate programs; maintain home and community-based services for individuals' age 65 and older and at risk of long-term nursing home use and/or spending down into medical assistance; and strengthen services for the caregiver support network. Models that serve older adults in rural areas are of particular interest.

Geriatrics Academic Career Award Program Inactive
Funding to support the career development of junior faculty in geriatrics as academic geriatricians or academic geriatrics specialists. The overall goals of the program are to develop the necessary skills to lead healthcare transformation in a variety of settings, including rural and/or medically underserved settings, to be age-friendly and to provide training in clinical geriatrics, including the training of interprofessional teams of healthcare professionals to provide healthcare for older adults.

Health Workforce Research Center Cooperative Agreement Program Inactive
