The program purpose is to reduce health inequity in areas with limited or no access to evidence‐based diabetes education, which include rural communities without a recognized or accredited Diabetes Self‐Management Education and Support (DSMES) program and populations that experience high rates of Type 2 diabetes, its complications, and diabetes‐related deaths.
