HealthStreet Cognitive Screening Project

Need: Because of the benefits associated with early identification of conditions causing memory problems, Florida's rural populations will benefit from access to screening for possible Alzheimer's Disease and other types of dementia.
Intervention: A state university uses a state health department grant to develop a cognitive impairment screening program implemented by rural Community Health Workers. An additional grant provides rural medical practitioners with a free online continuing education module covering cognitive impairment and dementia.
Results: To date, over 400 individuals have completed health screenings and over 900 referrals have been made to community social and medical services. At grant cycle completion, formal analysis of cognitive screening and referral to medical services will be shared.

Need: Mental health assessment and referral to resources for men in rural Michigan who struggle with depression and suicidal thoughts.
Intervention: The Healthy Men Michigan campaign was a research study testing online screening for depression, including irritability and anger, and suicide risk in working-aged men. The Healthy Men Michigan campaign website also provided referrals to local and national resources specific to men's mental health and suicide prevention.
Results: More than 5,000 individuals completed anonymous online screenings and 550 men enrolled in the study. Healthy Men Michigan secured partnerships with over 225 individual and organizational partners, including healthcare facilities, small businesses, and recreational groups across the state. Together, their efforts have helped to promote screenings, reduce stigma, and encourage help-seeking behavior to prevent suicide.

Need: Preventive and chronic medical condition care for farm families/agribusiness owners/employees with time and monetary constraints.
Intervention: Through a "Kitchen Wellness" program, a trained healthcare professional provides in-home or on-site complimentary preventive health screening, educational materials, and community referral information.
Results: Completion of hundreds of health screenings, tailored health education sessions and referral information.

Need: Connect individuals to services that address health barriers.
Intervention: A pay-for-outcomes model utilizing Community Health Workers who help provide community members with tools to address needs associated with improving health.
Results: Trained Community Health Workers help patients navigate the healthcare and social service systems and provide education about community healthcare resources.

Need: Comprehensive cancer services for residents of an 8-county, 3-state area in Appalachia.
Intervention: Using a Cancer Patient Navigation Tool Kit, a Maryland acute care facility led a multidisciplinary collaboration that provided the area's patients with expanded cancer treatment services.
Results: In addition to several new cancer-related programs, expanded services are now available for cancer patients, families, and cancer survivors.

Need: Meeting both advanced practice pharmacy student education needs and patient healthcare needs in a nearby rural/underserved area.
Intervention: With support from multiple organizations, students in the Ohio Northern University's College of Pharmacy program use a motor coach to deliver a wide range of healthcare services during scheduled outreach visits.
Results: In the program's first two years, point-of-care screening, immunizations, and chronic disease prevention and management education have been provided to 800+ Hardin County, Ohio, residents.

Need: Prevention of type 2 diabetes in adolescents living in rural parts of Louisiana.
Intervention: Through screenings, the Adolescent Pre-Diabetes Prevention Program detects the onset of prediabetes. Through nutrition and physical activity education, the program teaches high school students and staff how to adopt healthy lifestyles.
Results: The program has seen an increase in enrollment and continues to see decreases in body weight, body mass index, and A1C levels among participants.
