Cooperative Agreements for Innovative Community Crisis Response Partnerships Inactive
Funding to create or enhance existing mobile crisis response teams to divert adults, children, and youth experiencing mental health crises from law enforcement in high-need communities. A high-need community is one where mobile crisis services are absent or inconsistent, where most mental health crises are responded to by first responders, and/or where first responders are not adequately trained or equipped to diffuse mental health crises. This includes increasing access to licensed/credentialed professionals in rural and remote communities.

Improving Postpartum Maternal Outcomes for Populations Experiencing Disparities Inactive
Funding for clinical effectiveness research that compares multicomponent strategies to improve early detection of and timely care for complications up to six weeks postpartum among women in underserved populations, including women living in rural areas.

Training, technical assistance, and funding to help primary care clinics in Arkansas improve chronic disease management and prepare for Patient-Centered Medical Home (PCMH) accreditation.

Minority AIDS Initiative: Service Integration Inactive
Grants to integrate evidence-based, culturally competent mental and substance use disorder treatment with HIV primary care and prevention services. The population of focus is individuals of racial and ethnic minorities with a serious mental illness or co-occurring disorder living with or at risk for HIV and/or hepatitis.

Accountable Care Organization Realizing Equity, Access, and Community Health (ACO REACH) Model Inactive
Payment model to test financial risk sharing options, as well as innovative approaches from Medicare Advantage (MA) and private sector risk sharing arrangements. ACO REACH seeks to improve quality of care and health outcomes for Medicare beneficiaries through alignment of financial incentives to promote effective and appropriate care; the promotion of health equity among all model participants; emphasis on patient choice; strong monitoring to ensure that beneficiaries maintain access to care; and emphasis on care delivery for complex, chronically, and seriously ill populations.
