Addressing Diabetes Through Community Solutions for Healthy Food Access in Minnesota Inactive
Grants to improve health and reduce impacts of food and nutrition insecurity for people in Minnesota with or at risk for diabetes or prediabetes. Funding will support new or existing innovative models to help people reliably access high-quality nutritious foods, avoid hunger, and stay healthy. Priority populations include American Indians, and rural communities.

Mississippi Diabetes Self-Management Education Support Subgrant Inactive
Grants to provide training and capacity development to allow organizations in Mississippi to be recognized and certified by the American Diabetes Association (ADA) and the Association of Diabetes Care and Education Specialists (ADCES). The program purpose is to reduce health inequity in areas with limited or no access to evidence‐based diabetes education, which include rural communities without a recognized or accredited Diabetes Self‐Management Education and Support (DSMES) program and populations that experience high rates of Type 2 diabetes, its complications, and diabetes‐related deaths.
