The goal is to improve care provided to older adults in the emergency department, establish resources required to provide high-quality care, focus on providing the highest standard of care for older adults in the community. Hospitals in rural communities are encouraged to participate.

New York State Age-Friendly Health Systems Action Community Inactive
Learning community in New York that includes a virtual summit, monthly webinars, and topical coaching calls designed to incorporate evidence-based practices and transform the care given to older adults. Participants include Federally Qualified Health Centers and community-based social service organizations, among others.

Teaching Health Center Planning and Development Program Inactive
Planning and development grants to establish new accredited or expanded community-based primary care residency programs in family medicine, internal medicine, pediatrics, internal medicine-pediatrics, psychiatry, obstetrics and gynecology, general dentistry, pediatric dentistry, and/or geriatrics. The program is designed to support the expansion of the primary care physician and dental workforce in rural and underserved communities.
