Purpose

The Rural Program of All-Inclusive Care for the Elderly (PACE) Planning and Development supports organizations in the development or expansion of sustainable PACE programs to improve delivery of comprehensive medical and social services tailored to the needs of elderly individuals in rural areas.

Program goals include:

  • Provide start-up funding for organizations to develop a new PACE site serving the aging rural population
  • Provide funding for organizations to expand existing PACE programs into rural areas through service area expansion
  • Implement sustainable PACE programs that will effectively serve critical health care needs for rural aging populations

This program includes two funding pathways:

  • Funding Pathway 1: Initial PACE Program: This funding pathway supports new CMS applicants seeking to become a PACE organization in a rural area for the first time.
  • Funding Pathway 2: Service Area Expansion: This funding pathway supports the expansion of existing PACE organizations who are seeking to expand the service area to a HRSA-designated rural area. This includes an expansion of the currently approved geographic service area and/or the addition of a new PACE center site.

Applicants must have demonstrated experience serving, or the capacity to serve, rural underserved populations. Applicants are expected to prioritize improving delivery and access to PACE program services in rural areas. Rural service areas are defined as a county or census tract located in a HRSA-designated rural area.

Applicants must have demonstrated experience serving, or the capacity to serve, rural underserved populations. Eligible applicants include:

  • Higher education institutions
    • Public/state controlled institutions
    • Private institutions
  • Nonprofit organizations
    • With 501(c)(3) status
    • Without 501(c)(3) status
    • Native American tribal organizations
    • Community-based organizations
    • Regional organizations
  • Governments
    • State
    • County
    • City or township
    • Special districts
    • Federally recognized Indian/Native American tribal governments
    • Eligible agencies of the federal government
    • U.S. territory or possession
  • Healthcare facilities:
    • State and county health departments
    • Hospitals, including Rural Emergency Hospitals
    • Federally Qualified Health Centers
    • Rural Health Clinics
  • Other
    • Independent school districts
    • Small businesses
    • For-profit organizations
