Purpose

The Georgia State Office of Rural Health is offering funding to qualified organizations to offset costs associated with meeting Program of All-Inclusive Care for the Elderly (PACE) organization operational requirements. Applicants must verify a firm commitment and intent to complete and submit the full PACE application required by the Centers for Medicare and Medicaid Services (CMS) prior to the end of the funding period.

PACE is an integrated provider-based care intervention that operates under full financial risk for Medicare, Medicaid. It encompasses primary, secondary, and tertiary preventive acute care and an array of long-term services including home care, nursing home care, and advanced illness/end of life care.

Funds may be used for pre-operational staffing and pre-operational consultants/consultative services.

Eligibility

Eligibility criteria:

  • An established healthcare organization operating in Georgia capable of providing an all-inclusive care model for the elderly in a community-based setting
  • Currently providing long-term care services in Bibb, Chatham, DeKalb, Fulton, and/or Muscogee Counties
  • Nonprofit organization
  • In good standing to conduct business with the Georgia Department of Community Health
