Purpose

Health Center Program Expanded Hours funding is designed to expand access to health center services by increasing operating hours to meet identified patient and community needs.

Uses of expanded hours funds must align with the original H80 scope of project, which includes the approved service sites, services, providers, service area, and target population supported (wholly or in part) under the total approved health center budget.

Performance will be assessed based on the approved scope adjustments and documented increase in operating hours, as well as increases in:

  • Number of patients and visits
  • Hours across all service delivery sites
  • Number of sites with increased hours
