The Rural Changemaker Program provides funding to support grassroots work addressing community-identified challenges and inequalities related to health in the rural regions served by Interact for Health in Ohio, Indiana, and Kentucky.
Projects should:
- Center rural communities as the primary focus of a proposed or existing project or program, including leadership and decision-making power
- Make rural communities the primary beneficiaries of outcomes generated by the project or program
- Address a health challenge or underlying inequity identified by the community
- Plan, develop, implement, and/or sustain a solution
- Create action by energizing community members around a common, shared health concern
Eligibility criteria:
- Public or private nonprofit or governmental organization or entities with eligible fiscal sponsors
- Provide services in at least one county of Interact for Health's identified rural counties in Ohio, Indiana, or Kentucky

Award floor: $10,000
