The Office of Minority Health will provide grants for projects that demonstrate community level innovations to increase use of preventive health services, improve health outcomes, and reduce health disparities driven by social determinants of health.
Recipients will be expected to:
- Build or maintain a sustainable collaborative network to carry out the project
- Implement community level innovations that impact up to 2 Leading Health Indicators (LHIs)
- Evaluate processes and outcomes to assess project effectiveness and impact
- Share project findings, results, best practices, and lessons learned with the population(s) of focus, the public, and other stakeholders
Private nonprofit or public entities are eligible to apply, including:
- State governments
- County, city, township, and special district governments
- Independent school districts
- Public and state controlled institutions of higher education
- Federally recognized tribal governments
- Tribal organizations
- Public housing authorities
- Indian housing authorities
- Nonprofits with or without 501(c)(3) status
- Community- and faith-based organizations
Award ceiling: $600,000
Award floor: $475,000 per year
Project period: Up to 4 years
Estimated number of awards: 14
