Purpose

This opportunity will provide funding to recruit and deploy a group of community connectors in Minnesota that will provide culturally and linguistically inclusive outreach and navigation support for families as they enroll and participate in the Follow Along Program. Follow Along helps parents and caregivers of young children learn about their child's development and identify any needs, connecting them to supports and services based on those needs.

Focus populations include:

  • Tribal nations
  • American Indians
  • African Americans
  • Somali
  • Hmong
  • Latinx
  • Karen and other refugees from Burma
  • Rural communities
  • Socioeconomically disadvantaged communities
Eligibility

Eligible applicants include:

  • Community-based nonprofit and for-profit organizations
  • Government entities
  • Tribal governments
  • Clinics and hospitals
  • Community health and public health organizations
  • Quality improvement organizations
  • Professional provider associations
  • School districts
  • Institutions of higher education
  • Educational cooperatives

Applicants should have experience with:

  • Recruiting and training community members in providing culturally and linguistically responsive assistance to families navigating systems and services for young children
  • Hosting community events and facilitating conversations amongst parents and caregivers aimed at building family resiliency
  • Establishing connections with clinics and community organizations that serve families with young children
  • Using social media to conduct virtual outreach with diverse communities
  • Facilitating virtual and in person meetings and trainings with diverse communities
  • Completing evaluations and providing detailed reports on work
