The Integrated Maternal Health Services Program will provide funding to develop/demonstrate an integrated maternal health services model to support comprehensive care for pregnant and postpartum people who experience health disparities and have limited access to basic social and healthcare services.
The program will work towards achieving its goal by:
- Fostering collaborative relationships between healthcare providers, social services organizations/providers, state Medicaid programs, and state and local health departments to coordinate services and improve maternal health outcomes
- Developing and enhancing maternal health data infrastructure at the maternal healthcare practice and population levels
- Assessing the viability of various models that support integrated health services for pregnant and postpartum people
- Identifying lessons learned for future implementation
Eligible applicants are any domestic public or private entities, including tribes, tribal organizations, and community-based/faith-based organizations.
Award ceiling: $1,800,000 per year
