The High Obesity Program (HOP) provides funding to implement evidence-based strategies and leverage partner resources to address obesity and its associated factors in populations of focus related to poor diet and/or physical inactivity. Rural communities are considered a priority population.
Partners may include, but are not limited to, agriculture, transportation, education, business, healthcare, parks/recreation, community planning, and housing.
Required activities include:
- Promote food service and nutrition guidelines and associated healthy food procurement systems in facilities, programs or organizations where food is sold, served, or distributed
- Coordinate the uptake and expansion of existing fruit and vegetable voucher incentive and produce prescription programs
- Implement local level policies and activities to connect pedestrian, bicycle, or transit transportation network to everyday destinations
Optional activities include:
- Implement local level policies and activities that improve nutrition, physical activity, and breastfeeding and advance farm to early care and education (ECE) programs
- Collaborate with partners to implement family healthy weight programs
