This opportunity will provide funding to protect and improve the health and well-being of school-age children and adolescents in underserved and disproportionately affected communities. Projects should address social determinants of health and reach populations such as racial/ethnic minority students, students receiving free and reduced lunch, students of lower socioeconomic status, and those who live in rural/sparsely populated areas.
Expected outcomes include:
- Increase in the numbers of schools with an action plan based on the completion of CDC's School Health Index (SHI)
- Increased implementation of Comprehensive School Physical Activity Programs (CSPAPs)
- Increased school nutrition and food security policies and practices
- Increased physical activity, healthy dietary behaviors, and self-management of chronic health conditions among students
Eligible applicants include:
- Local, city, county, state, special district, and territorial governments or their bona fide agents
- Federally recognized native American tribal governments
- American Indian or Alaska native tribal governments (federally recognized or state-recognized)
- Native American tribal organizations (other than federally recognized tribal governments
- Government organizations
- American Indian or Alaska native tribally designated organizations
- Independent school districts
- Public, state controlled, and private institutions of higher education
- Public housing authorities/Indian housing authorities
- Nonprofit organizations with or without 501(c)(3) status with the IRS
- For-profit organizations
- Small businesses
Award ceiling: $425,000 per year
Award floor: $375,000 per year
Project period: 5 years
Estimated number of awards: 16
