The Special Diabetes Program for Indians program will provide funding for diabetes treatment and/or prevention activities/services for American Indian/Alaska Native (AI/AN) communities. Activities and services should be aimed at reducing the risk of diabetes in at-risk individuals, providing high quality care to those with diagnosed diabetes, and/or reducing the complications of diabetes.
Eligible applicants include:
- An Indian health program operated by a tribe or tribal organization through a contract, grant, cooperative agreement, or compact with the Indian Health Service
- An urban Indian health program operated by an Urban Indian Organization through a grant or contract with the Indian Health Service
- The Indian Health Service
Award ceiling: $600,000 per year
Award floor: $25,000 per year
Project period: 4 years
Estimated number of awards: 7-20
Estimated total program funding:
$40,000,000
Current awardees should budget for the same amount they received in FY 2023. New applicants should apply for a $25,000 base amount.
