Public Health Nursing Case Management: Reducing Sexually Transmitted Infections provides grants to mitigate the prevalence of sexually transmitted infections (STI) within Indian Country through a case management model that utilizes public health nurses (PHNs) as case managers. The program supports efforts to:

• Raise awareness of rising STI rates
• Increase access to care
• Strengthen surveillance systems to monitor STI trends
• Promote linkage to care in follow-up and treatment to reduce the spread of STIs
• Decrease serious health consequences of undiagnosed STIs.
• Identify effective interventions for reducing morbidity
• Improve outbreak response efforts

Applicants will first conduct a community health needs assessment to determine STI healthcare priorities in their community. They will then plan, implement, and evaluate program services.

Eligible applicants must be one of the following entities:

• Federally recognized tribes
• Tribal organizations
• Urban Indian organizations

Award ceiling: $150,000
Project period: 5 years
Estimated number of awards: 10
Estimated total program funding: $1,500,000
