Purpose

The Colorado Health Foundation provides funding for projects that directly engage community members to identify health challenges and implement solutions that meet the specific needs of their communities.

Organizations are required to work collaboratively with community members to:

  • Address a health challenge
  • Develop and implement a solution
  • Leverage existing strengths and resources
  • Improve health equity

Priority will be given to projects that advance health equity or that center on communities of color.

Eligibility

Eligible applicants include:

  • Community based groups or nonprofit organizations with 501(c)(3) status
  • Organizations that can demonstrate a substantial commitment to community-identified and community-led efforts to address complex health-related issues and/or challenges

Organizations without 501(c)(3) status may apply through a fiscal sponsor.
