Purpose

The Illinois Community Partners Grant provides funding to organizations to develop and implement strategies that reduce disparities and improve social conditions that impact health outcomes, especially in the context of ongoing challenges related to COVID-19:

Focus areas include:

  • Improve healthcare and its local environment
  • Enhance care coordination and access to care
  • Identify progressive solutions that consider social determinants of health, such as:
    • Housing
    • Employment, food security and other economic stability supports
    • Behavioral health
    • Anti-violence/safe environments
    • Infant/maternal health and reproductive health justice
Eligibility

In addition to aligning with one or more of the areas of focus, the following criteria are required for review of all grant proposals:

  • The organization must have a 501(c)(3) tax status for at least one year prior to application
  • Services provided in Illinois
  • Operates under a non-discrimination policy
  • Governed by an all-volunteer Board of Directors that are not compensated for their service

Amounts will range between $50,000 and $100,000.

Submit an online letter of intent by April 8, 2022. Selected organizations will be invited to submit a full application, with a deadline of May 27, 2022.
