The purpose of this funding opportunity is to support Health Center Controlled Networks (HCCNs) to advance the adoption and meaningful use of health information technology strategies and to leverage technology to deliver high-quality, culturally competent, equitable, and comprehensive primary healthcare.
The current funding cycle will focus on improvements in:
- Clinical quality
- Patient-centered care
- Provider and staff well-being
Eligible applicants must be either:
- A network that is at least majority controlled and, as applicable, at least majority owned by Health Center Program award recipients
- A current Health Center Program award recipient, funded for at least the 2 consecutive preceding years, applying on behalf of an HCCN
Maximum award amounts are based on the number of participating health centers, as listed in Table 1 of the application instructions.
