Michigan Community Health Matching Program
The Community Health Matching program offers grants to nonprofit community organizations that develop, implement and evaluate community-driven approaches to healthcare issues. Grants have been awarded in rural and tribal communities in the past.
Nonprofit 501(c)(3) organizations based in Michigan are eligible to apply.
Up to $25,000 per year for two years.
Matching funds are required based on the size of the applicant's budget and the percentage of Medicaid and uninsured patients it serves. For complete details, see the program website.

Applicants must submit a concept paper to foundation@bcbsm.com at least 60 days before the next grant review deadline to be considered for that funding cycle. Selected applicants will be invited to submit a full proposal.
Applications are accepted at any time and reviewed at Board of Directors meetings 3 times per year. Review dates and deadlines are listed on the program website.
