Grants for healthcare networks in rural areas to address gaps in service, enhance systems of care, and expand capacity of the local healthcare system. The purpose of this program is to support rural integrated healthcare networks that have combined the functions of the entities participating in the network in order to achieve efficiencies; expand access to, coordinate, and improve the quality of essential healthcare services; and strengthen the rural healthcare system as a whole.
