- Program Overview: The Southwest Montana Community Health Center (CHC) has integrated substance use disorder services within their existing primary care setting. Using evidence-based models such as screening, brief intervention, and referral to treatment (SBIRT), the CHC is able to link patients with mild substance use disorders issues to treatment, including referrals and counseling in a timely manner while in a primary care setting. In addition, the CHC established a community consortium with key partners and conducted a community-wide anti-stigma campaign.
