Geographic Disparities Associated with Travel to Medical Care and Attendance in Programs to Prevent/Manage Chronic Illness Among Middle-Aged and Older Adults in Texas
Examines 2,108 rural and urban middle-aged and older adults in Texas with one or more chronic conditions who travel long distances to receive medical care and their utilization of programs focused on prevention and chronic disease management. Participants from 9 counties responded to health assessment surveys regarding their number of doctor's visits within the previous year, distance to receive medical care, residential status, and other personal characteristics.

Gardening for Health: Using Garden Coordinators and Volunteers to Implement Rural School and Community Gardens
Describes a school and community garden intervention initially established by South Dakota State University (SDSU) Extension staff members with the support and collaboration of volunteers from schools and community organizations in rural areas and tribal organizations. Highlights how 13 community gardens in South Dakota became an integral part of a local community wellness coalition by improving local access to and consumption of fruits and vegetables. Evaluation of the Maryland All-Payer Model: Volume I - Final Report
Evaluation of the All-Payer Model operating under an agreement with the Centers for Medicare & Medicaid Services (CMS) for hospitals in rural and urban settings implemented in Maryland in 2014. This model shifted the state's hospital payment structure to an annual, global hospital budget that includes both inpatient and outpatient hospital services.
