CHAs identify concerns, set priorities, and include community profiles, demographics, and more. CHIPs and SOTCHs provide interactive scorecards for CHA-identified priorities. Also provides resources for the creation of CHAs, CHIPs, and SOTCHs in North Carolina, including checklists and submission process overviews.
