The Rural Outreach Center

Need: To expand services for individuals and families living in poverty in rural Western New York.
Intervention: The Rural Outreach Center (ROC) offers care coordination, therapy, budgeting assistance, and multiple other services and opportunities to help address immediate needs as well as empower individuals to work toward long-term freedom from generational poverty.
Results: The Rural Outreach Center serves approximately 250 adults and children each year through counseling, care coordination, and empowerment opportunities. The ROC reports that many participants have achieved and sustained goals related to housing, savings, employment, and other social determinants of health – which are also measures of poverty.

Need: To increase access to preventive dental care among children living in rural Arizona.
Intervention: A nonprofit organization formed to offer free, school-based preventive dental services to children in two rural Arizona counties.
Results: Throughout the 2023-24 school year, Tooth B.U.D.D.S. provided preventive services to 1,710 students in rural Graham and Greenlee Counties. Program staff use a telehealth platform to connect children to local dentists for follow-up care.

Need: To address the developmental, behavioral, and social/emotional needs of rural children ages 0-22 in northwest Illinois.
Intervention: Local partners teamed up to create a centralized service facility for children and families facing developmental, behavioral, and social/emotional issues. Florissa provides evaluations, diagnosis, and treatment to local children using a multidisciplinary, evidence-based approach.
Results: In 2023, Florissa provided a direct clinical service to over 337 children, in addition to many trainings and supportive programs. It also is co-located with the KSB Hospital pediatric department, a certified pediatric patient-centered medical home (PCMH).

Need: To expand public health and human services to adults living in rural West Central Missouri.
Intervention: HCC Network organizes Project Connect events, daylong resource fairs that offer free public health and human services to underserved adults.
Results: In 2023, Project Connect events welcomed 140 guests and offered numerous services and resource connections.

Need: To reduce youth suicide rates.
Intervention: First begun in Utah, Hope Squad is a nationwide program that trains youth to look after their classmates and refer those with suicidal thoughts or other mental health concerns to adult advisors.
Results: Studies suggest that Hope Squad schools' students with suicidal thoughts are more likely than non-Hope Squad schools' students to solicit help. In addition, stigma surrounding mental illness is decreasing.

Need: Address oral health disparities in Washington's rural and underserved communities.
Intervention: The University of Washington School of Dentistry (UWSOD) developed the Regional Initiatives in Dental Education (RIDE) program to increase the number of culturally and clinically competent dentists prepared for practice in rural and underserved areas.
Results: Since its start in 2009 and as of 2024, nearly 100 dentists have completed the program with over 81% practicing in rural and underserved communities.

Need: Clinicians in rural and underserved areas are often unprepared to provide comprehensive medical-forensic examinations for patients who present for care following a sexual assault.
Intervention: The MDPH TeleSANE Center uses secure telehealth software to connect sexual assault nurse examiners to clinicians and patients in hospitals across Massachusetts — including four in rural counties — offering expert clinical guidance and support before, during, and after examinations.
Results: Clinicians report that the service gives them increased confidence throughout the examination process. To date, the MDPH TeleSANE Center has assisted in the care of over 960 patients.

Need: To expand access to medications for opioid use disorder (MOUD) across the state of Pennsylvania.
Intervention: An initiative formed to provide technical assistance aimed at improving MOUD treatment capacity and quality at healthcare sites across urban and rural Pennsylvania.
Results: In 2023, 108 providers engaged with the program, treating a total of 5,185 individuals with MOUD. Currently, the program engages partners in 20 Pennsylvania counties – serving rural populations in Crawford, Schuylkill, Susquehanna, Butler, and Cambria counties.

Need: To create a safe space for youth and young adults, ages 14 to 25, in rural eastern Kentucky.
Intervention: The Sapling Center provides independent living skills education and offers a wide array of services as well as fun activities in a supportive environment.
Results: The 5 Sapling Center locations serve 50-75 teens and young adults every day.

Need: To increase access and quality of care for patients in rural Maine.
Intervention: Community hospitals and Federally Qualified Health Centers in Maine formed the Community Care Partnership of Maine Accountable Care Organization (CCPM ACO).
Results: CCPM serves about 120,000 patients in Maine. In addition, it has implemented shared savings arrangements/contracts with different Medicare Advantage and other private health payers.
