St. Francis Mission Dental Clinic

Need: To improve oral health among the residents of the Rosebud Reservation in South Dakota.
Intervention: St. Francis Mission founded a dental clinic, staffed by volunteers from across the country, to offer free oral healthcare to residents of the Rosebud Reservation.
Results: The clinic typically offers 20 one-week clinics every year and has served 3,000 patients since transitioning to electronic records in 2021.

Need: To address the specific health needs of north-central Oregon and south-central Washington.
Intervention: The Collective Impact Health Specialist identifies community needs, convenes community partners to design initiatives that address those needs, and secures funding for health-related initiatives.
Results: Thanks to the CIHS, the Columbia Gorge region has received $32.2 million since 2014.

Need: To increase access to healthcare and social services in rural South Carolina via library systems.
Intervention: Pilot sites are hiring social workers and community health workers to provide consultations, case management, and referrals.
Results: One library screens an average of 215 residents a month, and another library's programming reached over 800 female patrons.

Need: To expand services for individuals and families living in poverty in rural Western New York.
Intervention: The Rural Outreach Center (ROC) offers care coordination, therapy, budgeting assistance, and multiple other services and opportunities to help address immediate needs as well as empower individuals to work toward long-term freedom from generational poverty.
Results: The Rural Outreach Center serves approximately 250 adults and children each year through counseling, care coordination, and empowerment opportunities. The ROC reports that many participants have achieved and sustained goals related to housing, savings, employment, and other social determinants of health – which are also measures of poverty.

Need: To improve health outcomes in rural Appalachian Ohio.
Intervention: Schools as a Hub for Health promotes holistic wellness for the whole community by creating or bringing in programs that support physical, mental, and social health.
Results: The project has gained administrator buy-in and was featured in a December 2016 County Health Rankings & Roadmaps webinar.

Need: Suicide among veterans has been steadily increasing, and rural veterans have an increased risk of death by suicide compared to urban veterans.
Intervention: A program called Together With Veterans was formed to help rural communities address and prevent suicides among veterans. The initiative is veteran-led, collaborative, evidence-based, and community-centered.
Results: Data collection is ongoing.

Need: To lessen the impact of chronic disease and associated complications in North Carolina.
Intervention: Partners in Health and Wholeness, an initiative of the NC Council of Churches, is a faith-based program that integrates healthy living within congregations while offering financial support to launch or expand health initiatives. The program partners with faith communities in North Carolina to fund and support their health initiatives through collaborative partnerships.
Results: Since its founding in 2009, over 900 churches have become a part of the PHW, many in rural areas, and over 600 mini-grants have been awarded to congregations.
