Rural Health Care Services Outreach Program
The Federal Office of Rural Health Policy's Rural Health Care Services Outreach Program aims to expand healthcare delivery and address the health needs of rural communities by working with community partners and using evidence-informed models. In addition to the Regular Track, the program also includes the Healthy Rural Hometown Initiative (HRHI) Track, which focuses on the five leading causes of avoidable death — heart disease, cancer, unintentional injury/substance use disorder, chronic lower respiratory disease, and stroke.
