Addressing Diabetes Through Community Solutions for Healthy Food Access in Minnesota
Grants to improve health and reduce impacts of food and nutrition insecurity for people in Minnesota with or at risk for diabetes or prediabetes. Funding will support new or existing innovative models to help people reliably access high-quality nutritious foods, avoid hunger, and stay healthy. Priority populations include American Indians, and rural communities.

Funding to recruit and deploy a group of community connectors in Minnesota that will provide culturally and linguistically inclusive outreach and navigation support for families as they enroll and participate in the Follow Along Program. Follow Along helps parents and caregivers of young children learn about their child's development and identify any needs, connecting them to supports and services based on those needs.
